All concerned are hereby informed that the licensed firms, companies, vendors and service providers for the purchase and maintenance of goods and services as per the details required for this hospital in the financial year 2082/083 should obtain the details of the services and materials from the hospital's administration branch (Room No. 142) and register the list with the rates and documents as per the details in the same administration branch (Room No. 142) within 15 days from the date of publication of the notice.

Furniture and furnishing, steel and wooden goods supply and repair work

Computers, Computer Parts, Networking Services, CCTV, UPS.

Electrical, electronics, hardware, painting, plumbing

Oxygen and oxygen products

Lab Reagents/Supplies, Medical Consumables and Supplies

Pharmacy medicines, medical surgical equipment and consumables

Surgical implants

Work related to cloth, sewing and cutting

Cleaning products

Security, housekeeping, human resource supply

AC and equipment parts repair and supply work

Retail construction and repair work.

Documents that must be attached and sealed:

1. Application for registration of list

Firm or company registration certificate 4.

3. Tax payment certificate of last year

Value Added Tax Registration Certificate

5. Self-declaration that one is not disqualified or blacklisted under the prevailing law.

6. Rates of materials and services.

Again

The registration of companies without rates will not be valid. All rights regarding the registration of the list will remain with the hospital administration.
