Documents to be submitted with the application:

1. Copy of the certificate of registration of the organization, firm or company (including renewal details).

2. Copy of Permanent Account Number or Value Added Tax Registration Certificate.

3. Copy of tax payment certificate for FY 81/82.

4. Copy of business license as required.

5. A self-declaration that one is not eligible to be registered on the list and is not blacklisted, along with the application for registration on the existing list for public procurement work.

6. Details of the goods and services supplied, provided or distributed by the above-mentioned firm, company, service provider or supplier among the goods mentioned in the attached list.

7. Copy of certificate confirming that the distributor is an authorized dealer and distributor.

8. Details of directors or share register or details of proprietor.

9. Self-declaration that he/she is not involved in any crime related to money laundering.
