Information regarding registration for health check-up

Since there is a legal provision to conduct a mandatory health examination (health examination) for obtaining a new/class additional registration and renewal of a driving license in this office, for that purpose, the work has to be performed by charging only Rs. 15/- (fifteen rupees in capital letters) per service recipient in the fiscal year 2082/083. Therefore, in order to be listed, doctors, firms, companies or institutions recognized by the Government of Nepal to do the related work should submit an application to this office within 15 (fifteen) days from the date of publication of the notice for the first time, subject to the detailed conditions. This notice has been published for the information of all in accordance with Section 6 (a) of the Public Procurement Act, 2063. , Section 6 (a) of 2063

conditions

1. To be listed, the application must be accompanied by a certified copy of the registration certificate (including renewal) of the firm, company or organization performing health examination work, a certified copy of the business registration certificate, a certified copy of the VAT registration certificate, a certified copy of the tax payment certificate and, in the case of a doctor, a certified copy of the Nepali citizenship certificate, a certified copy of the permanent account number registration certificate, a certified copy of the tax payment certificate, a certified copy of the Nepal Medical Council registration certificate (doctors registered with a council other than the Nepal Medical Council must submit an official letter from the concerned council regarding the issuance of a health certificate), a written self-declaration that they are not disqualified for any reason, and a stamp of Rs. 10/- (ten rupees in words) should be submitted to the office.

2. To be listed, a deposit of Rs. 2,00,000/- (Two lakh rupees only) should be deposited in the deposit account No. 3020100202030000 of the Provincial Controller of Accounts, Gandaki Province, Pokhara, Rastriya Banijya Bank Limited, code number 3370239024 of this office and the original voucher should be submitted.

3. Details of work to be done: While registering and renewing a new/class additional driver's license, the work specified in accordance with the legal provisions regarding health testing as per Rules 25(2), 37(2), 42(2) and 44(2) of the Gandaki Province Vehicle and Transport Management Regulations, 2080.

4. Place of work: The listed doctor, firm, company or organization must provide services from the location designated by the office.

5. Working hours: The listed doctor, firm, company or organization must provide health examination services at the times specified by the office.

6. In order to be listed, a commitment letter to charge a service fee of Rs. 15/- (fifteen rupees in characters) per service recipient for health examination must be attached with the application. No additional service fee may be charged for any other reason. The service fee for health examination must be written in numbers and letters and posted at the examination site in a manner visible to all service recipients.

7. The listed doctor, firm, company or organization shall issue a health certificate in accordance with the Gandaki Province Vehicle and Transportation Management Regulations, 2080, Schedule-14, Rules 25(2), Rules 27(2), Rules 42(2) and Rules 44(2). The records of the issued certificates shall be kept and made available to the office as requested by the office.

8. The listed doctor, firm, company or organization must fully comply with the prevailing laws related to health examinations. Otherwise, it will be held responsible. 9

This office shall have the sole right to accept or reject the registration.

10. If there are many doctors, firms, companies or institutions to be listed, the office will have to provide services as prescribed through a rotating system.

11. If a listed doctor, firm, company or organization causes damage or embezzlement to the movable and immovable property of the office, the compensation or amount of the loss or damage shall be paid to the office.

12. The office may remove a listed doctor, firm, company or organization from the list at any time if the office no longer requires the services of the listed doctor, firm, company or organization, or if the services are not satisfactory, or if the office acts in a way that is detrimental, or if any objection is found in the documents submitted.
