Information regarding registration as an authorized supplier (seller) and submission of rates

For the purchase of chemical kits, proprietary goods and X-ray films used in the following machinery and equipment in various branches of this Provincial Hospital, Gaur in the year 2082/083, the companies/firms interested in being listed as per Section 41 of 1 (c) of the Public Procurement Act, 2063 and Rule 85 (6) of the Public Procurement Regulations, 2064 (including the eleventh amendment) are requested to submit their renewed firm registration certificate, VAT registration certificate, certificate of being the sole authorized supplier of the concerned company, authorized seller letter, certified copy of tax payment certificate of the year 2080/081 and details of submitting income statement to the Inland Revenue Department along with the rates of the chemicals, kits, proprietary goods and X-ray films to be used in the mentioned machines, along with the rates, within 15 days from the date of publication of the notice. This notice has been published for the information of all concerned.

1. If more than one authorized seller of the same company supplying the mentioned goods submits a price, the supplier who offers the lowest price after evaluation will be made an agreement and the goods will be supplied.

2. No action will be taken on any proposal that is not submitted in a proper manner.

3. For detailed information, you can contact the hospital's accounting or store branch.

4. The hospital shall also have the right to partially or fully accept or reject the form.

5. Copy of tax certificate for FY 2081/082, if applicable.

6. In case of other matters not mentioned in this notice, the Public Procurement Act 2063 and the Public Procurement Regulations 2064 and the prevailing laws of Nepal shall apply.
