Notice for Registration of Inventory

As a list of firms and companies has to be registered for the purpose of obtaining materials and services from licensed and interested individuals, firms or companies to supply the following materials and services required for this hospital during the financial year 2082/83, it is requested to register the application along with the necessary documents within 15 days from the date of publication of this notice.

Documents to be submitted with the application:

a) Copy of firm, company registration certificate.

b) Copy of Value Added Tax Registration Certificate or copy of income and expenditure submission certificate.

c) Copy of tax payment certificate for FY 2080/081.

d) Copies of documents proving work experience.

e) Details of the goods/materials or services to be supplied.

f) Copies of documents proving that the person is an authorized seller of the material/goods or services.

Goods/Materials or Services:

1) Stationery/Office Supplies

2) Medical and surgical instruments and all medical supplies,

3) Laboratory (Lab) related materials (Lab equipment, chemicals, reagents and all other materials) news

4) Electronic equipment for office purposes: computers, laptops, printers, scanners, fans, air conditioners, heaters, etc. and their parts and accessories.

5) Ink, ribbon, toner refilling cartridges, etc. for printers, photocopiers, etc.

6) Supply and maintenance of office furniture (wood, metal, etc.).

7) Maintenance of telephone systems/other communication equipment.

8. Office cleaning and housekeeping supplies.

9) Including supply and maintenance of interior decoration, flooring and furnishing items.

10) Printing and supply of stationery, various promotional materials.

11) Other miscellaneous materials and services.

A separate application must be submitted for each work for the supply of materials, maintenance and services mentioned above. Applications for list registration can be made via e-mail or directly to the Procurement Section in a sealed envelope. Applications that are not in order or arrive after the deadline will not be accepted. The hospital management reserves the right to make a decision in this regard.
