Outsourcing laboratory testing (must be an A grade lab)

(1) Application for registration of the list, (2) Registration certificate of the firm or company, (3) Tax payment certificate for the previous fiscal year, (4) Value Added Tax registration certificate, (5) Self-declaration that one is not disqualified or blacklisted under the prevailing law, (6) Rates of goods and services.

All rights regarding the registration of the list will remain vested in the hospital administration.
