Since a list of available items has to be prepared for the supply of various goods and services as mentioned in the details for the Central Department of Physics and Mathematics, Surkhet, Madhya Paschim University, in the fiscal year 2082/2083, the individuals, firms, organizations and companies who have obtained a license for the relevant work, who have reached the qualification level, are requested to submit an application for registration of the list to this office within the office hours of the 15th (first day of office opening if it is a holiday) day from the date of publication of this notice, along with the necessary documents, for the following goods and services to be supplied by them, in the format as per Schedule-2 (a) related to Sub-rule 1 of Rule 18 of Chapter 3 of the Public Procurement Regulations 2064. This notice has been published.

Documents to be submitted by the applicant:

1. In the written application, affix a stamp of Rs. 10, the firm and company, permanent account number, VAT registration, tax payment certificate for the fiscal year 2081/2082, and a copy of the citizenship certificate.

2. A written commitment letter to provide the requested goods and services on time must be submitted.

Other information

1. Applications submitted after the deadline, without a certified copy of the required certificate attached, or without the required form will not be listed.

2. This office may purchase goods or services as required from listed individuals, firms, organizations or companies.

3. If listed individuals, firms, organizations or companies do not supply goods and services satisfactorily, they may be removed from the list, and no claim can be made against this.

4. This office shall have full authority to list or not list individuals, firms, organizations or companies.

5. The above-mentioned existing list will be valid only for the financial year 2082/2083.
