This Hams Hospital Ltd. is required to register a list of individuals, firms and companies interested in supplying the materials and services mentioned in the schedule for the year 082/083, and therefore, the necessary request is made within 21 days from the date of publication of this notice. The application along with the documents should be registered in a proper manner.

(a) Name of the interested supplier, individual, firm, company:

(c) Copy of firm company registration certificate

(d) Copy of Value Added Tax Registration Certificate

(e) Copy of tax payment certificate for FY 080/081

(f) Details of the goods/materials or services to be supplied, including rates.

(g) If the supplier is an authorized seller of the goods or services, a copy of the document proving that

(h) Details of total transactions for FY 075/080 and 080/081.

Individuals and companies interested in supplying the above-mentioned materials and services may submit separate applications, including details of the goods, rates, and other information, to the Procurement Section of this hospital through e-mail or in a sealed envelope. Applications that are incomplete or have exceeded the deadline will not be considered. The management of Hams Hospital reserves the right to make a decision in this regard. cal news
