We request that interested firms and companies register a list of firms/companies approved to transact the relevant goods and services for the purpose of supplying/providing the goods and services mentioned in the schedule required for the year 2082/083 to Grande International Hospital Limited by submitting the documents mentioned below and the form available on the website of this hospital: www.grandehospital.com during office hours within 15 days from the date of publication of this notice and sending the list to Email: [email protected] or by submitting it to this office.

1. The following documents must be submitted with the application.

Certificate of Company Registration

Permanent Account or VAT Registration Certificate

Tax Payment Certificate (A.Y. 2080/081)

Authorized Seller Certificate (only if applicable)

Registration certificate from the Department of Drug Management (only in the case of drugs)

2. You must submit a quotation for the goods and services you wish to distribute within the time of registration.

3. The hospital will enter into a separate purchase agreement with the firm to be listed.

4. Applications that are not received within the specified time and that are incomplete will not be accepted.

5. The hospital reserves the right to list or not list the applications received. aa
