In this Fikkal Hospital Ilam, for the purchase of kits, chemicals, films, accessories and spare parts used in the medical equipment and machinery as specified in the details and consultancy services related to maintenance and maintenance as proprietary goods (Closed Items) in the year 2082/083, the authorized sellers (proprietors) of the concerned manufacturing companies are requested to register as per Sub-section 1(c) of Section 41 of the Public Procurement Act 2063 and Sub-rules 3 and 6 of Rule 85 of the Public Procurement Regulations 2064 by submitting the following documents: Firm Registration Certificate with renewal, VAT Registration Certificate, Self-declaration that they are not eligible to participate in the procurement process, Business Registration License, Business Registration License, etc. This notice is published to submit proposals for listing within fifteen days (2082.05.16 at 2:00 PM) from the date of publication of this notice, mentioning the company/firm to be listed, along with the technical and financial proposals for the kits, chemicals, films and main parts (System Pack Reagent and Disposal) to be used in the machines mentioned in the details, along with copies of the tax payment certificate of 2080/081 and the official seller certificate of the concerned company (Authorization Letter up to the supplier company including the manufacturing company) in separate sealed envelopes and then sealed in another envelope and addressed to Fikkal Hospital Ilam, mentioning the financial/technical proposal and mentioning the company/firm to be listed. In case more than one official seller of the same company submits proposals/rates for supplying the mentioned kits, chemicals, films and main parts, arrangements will be made for the supply of the goods by making an agreement with the supplier who offers the lowest rate. No action will be taken against applications that do not submit the official documents requested in the notice, are not in due time and are received after the deadline.
